Can the base use an AC wall adapter or is it batteries only?
It is batteries only, 3 AAA batteries, with no jack for an adapter. However, battery life is excellent; I purchased mine on April 19, 2015 and I have not had to replace my batteries yet. I just tested them and they are still going strong. However, we haven't had to actually 'use' it much, fortunately.

Press and hold the signal button for 3 seconds until both lights come on. Then press each signal button and see if the corresponding light starts to flash. Then they should be paired. If that doesn't work, I would check the batteries if the call buttons.

I love this device and the lights blink when the batteries get low but every 5 days? This last 5 days it wasn't even used. Anyone else?
I sent mine back and ordered a different device, Safty First. Works so much better. Haven't changed a battery in 3 months.

Can the two call buttons be used in by different clients in two different rooms so that if pressed, I can tell which room is calling?
Yes, there are different tones and lights so you can tell if it's red or blue.

I live across the street from my grandparents. Will this work with the distance between us?
The max distance would have to be less than 500 ft. If that's the case, it should work fine. I would use it while walking to the market down my street and it was fine.
